Verified Petition for Change of Name in the
above-entitled Court praying that said Court
enter an Order changing Petitioner’s legal
name from Melissa Dee Muir to Melissa Dee
Gerrard. Any person having objection to the
changing of Petitioner’s birth name must file
a written objection in the above-entitled Court
within 10 days after the date of publication
